Author information: (1)Kansas City University (2)Walter Reed National Military Medical Center A boutonniere deformity is characterized by flexion of the proximal interphalangeal (PIP) joint and hyperextension of the distal interphalangeal (DIP) joint. Etiologies include acute injuries, such as forceful PIP flexion, dorsal lacerations, or full-thickness burns near the PIP joint, as well as chronic processes, including progressive capsular distension in the context of rheumatoid arthritis. All mechanisms produce disruption of the central slip of the digital extensor mechanism, resulting in volar migration of the lateral bands and formation of the characteristic boutonniere deformity (see Image.
