Lateral elbow pain is a highly unspecific finding and may be caused by one of several possible differential diagnoses. Unfortunately, it is often assumed to be epicondylitis, commonly referred to as tennis elbow, although instability is a frequent cause in chronic cases. In the presented case, it is the less frequent nerve compression of the radial nerve which must also be considered, together with plica syndrome, osteochondritis dissecans and arthrosis of the radiocapitellar joint, as a differential diagnosis of lateral elbow pain. Publisher: Zusammenfassung. Der laterale Ellbogenschmerz ist ein höchst unspezifischer Befund, hinter dem eine Reihe möglicher Differenzialdiagnosen stecken können. Leider wird hier oft «reflexartig» von einer Epikondylopathie, umgangssprachlich Tennisarm, ausgegangen, obwohl gerade in chronischen Fällen eine Instabilität viel häufiger ist. Im vorgestellten Fall handelt es sich um die seltenere Nervenkompression des N. radialis, die gemeinsam mit dem Plicasyndrom, der Osteochondrosis dissecans und der Arthrose des radiokapitellaren Gelenks als Differenzialdiagnose des lateralen Ellbogenschmerzes bedacht werden muss.
