Mallet finger injury of the distal extensor mechanism of a digit represents a spectrum of pathology. The hyperextension variant, characterized by a major intraarticular fracture fragment and volar subluxation of the distal phalanx, requires specific recognition. Its distinctive mechanism of injury requires modification of routine mallet finger management, whether open or closed.
