Gunshot wounds can produce injuries ranging from small low-energy soft-tissue wounds to devastating open fractures with vascular, nerve and soft-tissue damage. Management is determined by the actual tissue injury, contamination, fracture stability and associated neurovascular or visceral injuries rather than by projectile velocity alone. Uncomplicated low-energy fractures may often be treated according to standard fracture principles, while high-energy injuries usually require open-fracture management with debridement, antibiotics, skeletal stabilisation and soft-tissue reconstruction. Retained bullets do not require routine removal, although intra-articular, symptomatic, infected or neurovascularly threatening fragments are important exceptions. Careful neurovascular assessment, early recognition of compartment syndrome, appropriate tetanus prophylaxis and coordinated rehabilitation are essential for optimal functional recovery.
